Motiva breast implants are officially available at La Jolla Cosmetic Surgery Center, and we're absolutely thrilled to be offering these revolutionary breast implants. And you might want to know what makes it interesting, why are they revolutionary? And so I have Dr. Diana Breister here to introduce them and tell us a little bit about what makes these implants different and special. So welcome back to the podcast, Dr. Breister.

 

Dr. Breister (00:30):
Hi, good morning. Thank you for having me.

 

Monique Ramsey (00:32):
So I want to start off just by giving everybody a little bit of advice. If you're listening to us, that's great. But you can watch us on Spotify and on YouTube because we're going to have a little show and tell here with these breast implants. So if you want to switch now, it would be a good time, cuz we will go ahead and get into the ins and outs of these implants, and it is a visual thing. So, alright, so let's start with what makes Motiva implants special?

 

Dr. Breister (01:02):
Well, this is an exciting day. We're very excited to have these implants now available in the United States. They're an implant, they have been used in Europe for the last 10 to 14 years, so they're definitely not brand new. So that's the one thing I want to clarify. If people are scared, like, oh, it's a new device, so we don't know a lot about it, that's not the case. We know a lot about it from our colleagues over in Europe who've been using it. So that's number one. Number two, there's quite a few features of it that are very special and very exciting, and as surgeons, we just can't wait to be able to use this because ultimately it can lead to less complications. So one of the biggest problems in breast augmentation can be the formation of a capsule. So that's essentially scar tissue, but that scar tissue in some women can become hard or pathologic.

 

(01:53):
The greatest, I think, positive, exciting thing about this new implant is that the studies, at least that we have seen, and they are not extremely long-term, but they're at least about four years, show an extremely low incidence of this capsular formation. So that is the A, number one really, really exciting part about it, is that it has a shell that has a slight bit of texture, and they have figured out that slight bit of texture matches kind of our body's texture. So therefore our body doesn't really have to have too much of a response towards it, cuz it's so similar to ours. So because of that, what they've shown is that the incidence of encapsulation is less than 1%, which if it truly pans out over time would be absolutely phenomenal. Because right now the capsulation rates range anywhere from about eight to 20%. So that's going to be really, really exciting, is that offering a device that has a less incidence of that is just fantastic. And amongst that, there are many other reasons, and I can get into the scientific theories of why that is, or we can kind of stick to the basics.

 

Monique Ramsey (03:14):
Now, I'm intrigued. So tell me a little bit about the science.

 

Dr. Breister (03:19):
Okay. So the generation of silicone is a sixth generation silicone. So they've basically kept improving on the cohesiveness of the implant and the gel inside, but the texture of that shell is four microns of texture. So a smooth implant is one micron of texture, so that's the one that can form a capsule. The more textured implants, those were popular for a while and they were good because they had less capsule. However, there's a small incidence of a cancer that can form in that. So therefore most of us don't want to use a truly textured implant, and those are usually about 10 microns. This new smooth silk implant, I'm just going to kind of show one, it's considered a smooth implant, but like I said, there is about four microns of texture there. As we said, that causes less of a reaction of our body. The other exciting scientific feature is going to be the way they have formed the capsule of the implant.

 

(04:30):
So we've got gel inside and then we have a shell around it. So this shell has properties that are going to be, I think, better for the incidence of rupture as well. The science and the studies have shown a very low incidence of implant rupture that also along with capsular contracture, we want the lowest incidence of possible implant rupture. So as we can see from right now, this is the one that has a very low incidence. And in those studies also less than 1% over four years. So the shell properties, the gel properties, the texture are what really make this exciting and possibly safer and more reliable than the other implants that we're using. And then we can get to the exciting differences. They offer two different types of implants, and those are basically different in the softness of the gel. One is called the round, and it's a little bit, I don't want to say firm because it's not firm at all, but it's firmer than the other one. So the round implant has a really nice gel, and if you can kind of see here, it holds its structure. If I'm holding it on the side, you can see that the gel kind of stays in this form. So this can give women a really nice upper pole fullness. It's wonderful to accomplish that.

 

Monique Ramsey (05:59):
Upper pole fullness is that kind of cleavage?

 

Dr. Breister (06:02):
Exactly.

 

Monique Ramsey (06:03):
People are looking for?

 

Dr. Breister (06:06):
Women, we all want something different. Some just want a natural yoga breast, somewhat really, really full medial cleavage. And obviously our job is to get the patient what they want. So this can lead to a little fuller of a profile, a little fuller cleavage. Okay. The second implant, they call this an ergonomix implant. And what we're very excited about is that the gel is very, what they call visco elastic. So it's very stretchy, yet cohesive, and it is supposed to move with your body. So if you're moving around, this gel goes along under your breast tissue and is more natural per se. Now, it's not like the other one's that unnatural, but this is kind of more of what we call a teardrop. So many, many women will come in and say, I want the teardrop, I want the teardrop. This allows a woman to essentially get that shaped or anatomic implant without having to have the vigorous amount of texturing.

 

(07:10):
So this is going to be a good implant for the woman that really, really, really, really wants to look extremely natural. This is just going to be a little softer and a little more movement with her body. Once again, these implants are merely tools for us, and we have a toolbox of things, but the most important part of obviously this process is having the doctor who analyzes what you want, analyzes your body type, what kind of breast tissue do you have? What are you looking for? A woman with thicker, robust tissue is going to have a different set of things we need to think about than a woman who's very thin and doesn't have a lot of subcutaneous fat. So those issues all play a part, and these are merely another exciting tool that we have into accomplishing a woman's goals and in the safest way possible.

 

Monique Ramsey (08:07):
Now, I heard that they're kind of stretchy and does that stretchiness, I guess that makes that movement more natural, but is it also easier to insert?

 

Dr. Breister (08:19):
Well, yes, that too, since it is very pliable and we can squeeze it really tight and get it in through smaller incisions. So that is also a plus of it, is that we can make the little incision a bit smaller because it is so moldable and squeezable and get it into that small area without making as large of a scar as before.

 

Monique Ramsey (08:44):
And I love that there's something new in the breast implant world, and I love that, like you say, it's a toolbox thing. We have multiple implants depending on what you're looking for, what your body dictates. And during a consultation, are patients allowed to feel the implants?

 

Dr. Breister (09:03):
Oh yeah. We bring these out, we show them all the different sizes, different shapes, explain to them how their body might require one over the other. It's quite a process, and it's something that I think patients being explained, the nuances of these things and the nuances of their body, it really helps them to build confidence that the doctor's really listening to them. But these implants super exciting. I can't wait to start using them. I've done several of them now and I really, really love them. The patients have had great, so far, so good. They're doing well. There's more sizes that are going to be coming out. So we don't have the huge range of some of the others. So some of the very large sizes we don't have yet, but we have a nice array. And like I said, it's just in the last several months that the FDA has approved this and we've been able to get our hands on it and start using it.

 

Monique Ramsey (09:59):
